It makes sense to speak of enhancing care because the care being offered
to bipolar patients is currently less than it should be. The basics never
change, of course: doctors should take a careful history, conduct a
relevant physical examination and make the necessary investigations to
formulate their diagnosis. This, and the medical plan of management,
should be explained. Communication must be clear and effective. The
objective is a therapeutic alliance between doctor and patient, which
is essential for the management of a complex chronic condition.
Bipolar patients require more than this, however, because they must
assume such a vital self-management role in an illness that can distort
perception and judgment. Most succeed only by acquiring knowledge,
experience, skill and the ongoing collaboration of friends and relatives.
Clinicians can foster success in this endeavor by using a systematic
approach to deliver individualized measurement-based treatment. This
approach begins with recognition that bipolar disorder is a chronic
condition that will be managed over time with a series of interventions.
The long-term goal is to maximize the patient’s quality of life by
increasing the rate at which patients and clinicians produce wise
amicable decisions. The simple schema we offer to achieve this aim
(Figure 5.1) requires both clinicians and patients to have a base of
knowledge and skills related to bipolar disorder and its management.
Their working relationship includes integration of routine measurement
into a shared management process.

Once a patient accepts the broad facts about bipolar disorder, they
give up a sense of personal responsibility for at least part of what the
disorder is about. However, there is a new challenge, which is to identify

Adherence to medicines
Patient outcomes can be improved by the measures described above for
enhancing care, but this approach is not an alternative to long-term
treatment with medicines. Moreover, no medicine can work if a patient
does not take it. Whether this problem is viewed as non-adherence, poor
compliance or a lack of concordance between patient and care provider,
it is common across many chronic illnesses and should be addressed as
openly and amicably as possible. Bipolar disorder is no exception: up to
50% of patients are non-adherent with treatment. Hence, the response
to a course of treatment with inadequate duration or dosage is
inconclusive. The progression toward wise decision-making follows
from definitive classification of the outcome for each intervention as
effective, ineffective or intolerable. Thus, adherence to prescribed
medicines is a key issue.
Given the profiles of the existing medicines, it is often assumed that
adverse effects are a major reason for early discontinuation. However,
data from the UK suggest that only about 15% of non-adherence
is attributable to adverse effects (Figure 5.3). The majority of non-
adherence appears to occur because patients do not understand the
need for treatment or do not want treatment. Reaching agreement (or
concordance) with patients on these crucial points as early as possible
may be the most salient therapeutic objective in the long-term
management of bipolar disorder. 57

improve adherence and, indeed, that this may account for a large part of
the improved outcome associated with psychological approaches
(Figure 5.4).
Cognitive–behavior therapy
Cognitive–behavior therapy (CBT) is widely promoted as an approach
to the treatment of mood disorder. It was developed from Beck’s specific
formulation of a cognitive model for depression. It emphasizes the
potential for thoughts and memories to influence mood, rather than vice
versa. Its focus is on conscious cognitive biases that may lead patients
to make negative interpretations of potentially neutral or even positive
events or difficulties. A central ambitious objective is to ‘restructure’ the
patient’s negative cognitions.
