STUDENTS AT THE SCHOOL OF NURSING SCIENCES OF THE UNIVERSITY OF
NAMIBIA MAIN CAMPUS

As a Bachelors in Nursing Science (clinical honors) student at the University of Namibia, I am

undertaking a research to assess the awareness and beliefs regarding the female condom amongst
the students at the school of nursing. I will be grateful if you would complete the following
questionnaire and return it to me.

Please complete as many of the questions as honestly as you are able to. This questionnaire is
entirely anonymous and all the information collected will be treated with utmost confidentiality.
As such you do not need to put your name or any identifiable information.
Completing this questionnaire, will afford you a chance to aid in finding possible solutions
which might increase the awareness of the female condom and consequently its uptake and as a
result thereof, help combat sexually transmitted infections, at our university.
