SMSA “ The Hope “ Basketball Tournament

The hope tournament is a fundraising tournament to help raise money for the
national kidney society. Also it is helping to promote exercising and eating right on
a daily basis. This tournament will be held on April 6th after school. The games
will be 3 on 3 with one sub for each team. Shirts will be provided and food will be
available after the event for those participating. To enter a team you must have a
full team of four with 5 dollars from each player. Also, each player will need a
parent and the nurse to sign this paper. Turn this form back into Kalissa Jameson or
Mr Mckay.

To enter the tournament must have the following:

● A team of 4 students from SMSA Highschool
● Sign and return this permission slip with the fee of $5
● Must have your parent and the nurse sign off
● Team name and color shirt of choice.
