AFFIDAVIT OF HEIRSHIP

I, _____________________, of legal age, single and residing at #________________________

City, Philippines, after having been sworn in accordance with law, depose and say:

1. That I am the daughter and one of the surviving heirs of the late
________________________, who during her lifetime maintained an account with Air ;

2. That I attest to the fact that the following are the LEGAL HEIRS of the late PAULO
FORAYOG AYASAO:

MADELYN HABBILING AYASAO WIFE

MARK LOID HABBILING AYASAO, 9 years old SON
LEO PAUL HABBILING AYASAO, 6 years old SON

3. That as evidence of legal relation to the deceased, photocopies of our Marriage Contract,
Birth Certificate of Mark Loid Habbiling Ayasao, and Birth Certificate of Leo Paul
Habbiling Ayasao are herein attached; and

4. That I am executing this Affidavit of Heirship for myself and in behalf of my children,
who are minors, in relation to the Repatriation of the Remains of my late husband in
Jeddah, Kingdom of Saudi Arabia.
