Note: This list only includes known sites that conduct PCR (polymerase chain reaction) testing for actively
infected individuals. Each health care provider at each location will determine if testing for COVID-19 is
needed. Not all patients wanting a test will get one. All sites are by appointment only. This list is subject
to change based on testing availability.

DO NOT SHOW UP AT ANY LOCATION UNLESS YOU HAVE AN APPOINTMENT.
