Reference your offer against Tender Enquiry for purchase of Bio Medical & Lab.
Equipment’s and Hospital Furniture etc during the current financial year 2018-19.

2. The following rate has been approved by the Competent Authority subject to the condition
that you will deposit Performance Security in the form of an Irrevocable Bank Guarantee / CDR equivalent to
5% of the total contract amount having validity of five years from its date of issuance from any scheduled
bank on the prescribed format and in prescribed manner in favor of Chief Executive Officer District Health
Authority Toba Tek Singh, and provide stamp paper (s) as stamp duty @ 25 paisa per every 100 Rupees
of the total value of the contract (0.25% of the total value of the contract) under section 22(A)(b) of Schedule
1 of Stamp Duty Act 1899 read with Finance Act 1995 (Act-VI of 1995) for agreement within 07 (Seven)
Days from the date of issuance of this Notification of Award so that the contract under the rules may be
