too low so I got a question by one of my

viewers and apparently he's had a fairly


low score in the USMLE step one and I
believe he is a foreign medical graduate
so that doesn't help we are for medical
graduate and you have low scores and his
score to be exactly is is 195 so he's
asking if this is it for for him and he
basically is asking that he possibly can
get a postdoctoral fellowship research
position and that he could possibly do
he can possibly do that for one year
right now I think the the the correct
question for the moment for him is at
least what should be his strategy I
think he still wants to go for the USMLE
assembly and going to get a residency
and I think that that's fair that's
pretty fair to be honest I've had seen
people with fairly low scores in the
u.s. Emily and actually get residency
difficult it's going to be it's already
difficult to being an IMG to begin with
Howard it's still going to be a bit of a
bit of a struggle however I've seen good
things happen and depends on how it ends
up happen with you so what is going to
be your strategy let's take it from here
you have to I believe you should score
highly in your USMLE step two you should
hold off with your research for now you
should make sure you're going to get to
50 to 40 plus in your step 2
exam that that you have to do that you
have to do then all else comes second I
think you should hit the books and get
an X in school and that you assembly
step 2 that's number one priority next I
think for matching purposes you will
need to have step 3 out of the way get a
good score and you're--you assembly step
2 and immediately within two months and
within a month hit the books hard do eww
for step three and the cases and get
that out of the way as soon as possible
and pass the exam you have to pass you
cannot you cannot
cannot flunk step three step two
excellent score step two and pass step
three I think that's what you should be
should be doing next comes what you
would probably need to do you would need
to gain some u.s. clinical experience
preferably hands-on externship or type
of program observer ship use any
contacts you have pay any programmers
you have to you gotta get some US
clinical experience and letters of
recommendation that are going to be good
and personal recommendation letters I'll
have a video soon about how to get
personal letters of recommendation so
that that better look a difference when
anybody is reading your residency
application a generic recommendation
letter it's everybody everybody has that
everybody can make that up you need a
good personal recommendation letter and
that will put you stand you apart from
the rest of the crowd next I think this
should be good get a couple of good
recommendation letters do around six
months of clinical experience in the US
and I think that should be good for you
next what you should focus on is publish
whatever you can possibly can doing as
you mentioned that you are good you may
be doing a postdoctoral fellowship for a
year I think that that's excellent and
you can look at my previous video where
I talk about how to get the best out of
research or research for more
information how you could possibly do
that but for research you need posters
abstracts and then talks that's what you
that basically would count just hanging
out with somebody and trying to do some
data gathering is probably not going to
cut it but that's what research research
could help you
next you have to you have to see where
you can apply if you go for a surgical
surgical
a residency as a four American graduate
of the law schools I think you have a
difficult time even with medicine you'll
have a stream Li tough time but you
should look for programs in Family
Practice pedes medicine psychiatry I
think are all great and they will be
able to look at you if you have a
excellent school on your step two and
you have passed your step three and of
course you should not be having any
attempt in CS so just be careful you
have to have the mindset and if you go
strong I think you should be able to do
it next for you to consider there all
those programs that are unmatched every
year it's available on the available on
the n RMP a website you can look at
programs that are left and mash it could
be even surgical prelim surgery prelim
medical contact all those programs in
advance okay tell them hey you're you're
available and you will be able to fill
up these these programs if for some
reason they are unmatched and you are
very eager to join them I think that
that's something you should always also
be doing along with your other
preparation next if for example you do
get you do get your externship your us
clinical experience of whatever nature
or your research position you should be
getting your contact you should be
networking almost every single day
however at the same time you have to
show that you are the person they want
you you're smart you're intelligent you
have initiative you just had hard luck
and your USMLE step 1 don't even bring
that up for now sure what you are doing
a good job in doing rounds be be
inquisitive be eager be knowledgeable
know about your patients have good
present a good patients at presentations
make sure you formulate your assessment
and plan your differential make sure
that you get along with make sure you
get along with everybody and then you
will of course
naturally be liked by your attending so
keep on making contacts find more
contacts if you have in research to
spin-off projects as I mentioned my
other videos and and and then once you
have developed a good good good
collaborative connection with the people
then ask them for for their for for the
assistants and say hey you know I do a
good job I would love to see if you can
pull out an interview for me here and
there and I think they'll be okay if
you're a good person if you're able to
produce produce quality cool clinical
work or produce or VOC or work well with
your research project I think nobody
will have a problem everybody is looking
for good people everybody's looking for
people who are determined and
hardworking and and basically good
people to work but so I think that
should work for you so in your situation
you have to apply apply the program
broadly and make sure make sure that you
have have to contact these programs
before the applications season all right
a lot of most most candidates they will
be applying to they will be contacting
programs after the application has been
submitted what you have to do is you
have to talk to all those programs
before because you have ever tells me I
have an excellent school in my USM is
step two step step three is passed and
CSS is also passed and I'm all set to go
and if you have filters for low step one
scores please disregard that and get me
that interview and I have all this other
experience I have this elusive
experience and I have basic externships
and observer ships so get me and get me
that interview and and and basically I
just had hard luck in my family step one
so I think that should be it it's going
to be a little more difficult you have
to you know you'll have to you'll be you
sweat a little more than other people
but just to rephrase what you
have to do from now on you assembly step
to pass with excellent score then you
simply step three you got up you got to
get that you got to get that in the bag
before you apply for residency all right
it cannot be pending it cannot depending
next you need to have us clinical
experience and good recommend
recommendation letters what is this
next you have to you have you have to
have some type of research if possible
kiss force post representations abstract
submissions you've got to have that then
you got a you got to make sure you
choose your programs wisely don't don't
fix yourself on just doing telemedicine
or any other specialty brought apply
broad family practice pedes any things
you can possibly get your hands on a
prelim surgery anything at all next you
got a network you get a networks you
have to be the persons you have to be
confident individual that people want
talk to and and try to get people to
understand that hey yeah you you are the
person that they basically want and you
should play it like that I think that
should be fine nothing's over I've seen
people with low scores getting to 40 to
50 plus and and I've seen it and I've
seen it and it makes it does with your
low score your job a little difficult
but I think you should be able to do it
