Environmental Compliance Certificate (ECC) Application

A. Certification from LGU on the compatibility of proposed project with exiting

land use plan in the area (Locational/Zoning Clearance)
B. Vicinity map of the proposed project signed by registered professional
(Geodetic/Civil Engineer)
C. Project Layout Plan signed by registered professional (Civil Engineer/
Architect)
D. Proof of authority over the project site (Copy of land title, deed of sale, tax
declaration and/or lease contract, related land consolidation report from DAR
and/or DENR)
E. Affidavit of no complaints (Project endorsement from barangay and municipal
LGUs and affidavit from lot owners, if any)
F. Project Description (PD)
G. Project Components and Operation Information (PCOI)
H. Environmental Impact and Management Plan (EIMP)
I. Project Environmental Monitoring and Audit Prioritization Scheme (PEMAPS)
J. Schematic Diagram of wastewater treatment facility
K. Schematic Diagram of air pollution control facility
L. Organizational Chart of the project in-charge of environmental concerns
M. Abandonment/decommissioning/rehabilitation plan
N. Topographic map of impact/affected areas
O. Geotagged photographs of project site
P. Sworn Statement of Accountability of the Proponent
Q. Payment of Application Fee (Php5,050.00)

Note: Items A to E and Q are basic requirements to be secured by the project

proponent and to be provided to EIA preparer for packaging as part of the
Initial Environmental Examination (IEE) document.

Item F will be provided by the proponent to the EIA preparer as source of

useful technical and operation information needed in the preparation of the
IEE.

Items G to P are EIA preparer’s responsibility.
