AFFIDAVIT OF CANCELLATION OF MORTGAGE

I, ______________, of legal age, Filipino citizen, married and a resident of Brgy.

_____________, Philippines, after having been duly sworn to in accordance with law, do
hereby depose and say:

That I am the Mortgagee of Lot No. 1234 located in Brgy. ________ and covered
by ARP/TD No. 1234567 (2020) and declared in the name of __________ which was
mortgaged by the latter to me for and in consideration of the amount of FIFTY
THOUSAND PESOS (Php 50,000.00) as annotated in the aforesaid tax declaration dated
JANUARY 2, 2020;

That for and in consideration of the amount of SEVENTY THOUSAND PESOS

(Php 70,000.00) paid in hand to me by ___________, buyer of the aforesaid parcels, I
hereby CANCEL the mortgages as annotated in the tax declarations of the above-
mentioned parcels of land. That I no longer have any interest or claims whatsoever over
the same;

That I am executing this affidavit to attest to the truth of the foregoing facts and in
order for the buyer, ____________ to process the transfer the ownership of the above-
mentioned parcels of land in her name before the Municipal Assessor, Provincial
Assessor, Bureau of Internal Revenue, Department of Environment and Natural
Resources and other government agencies concerned.
