No. : BCPRD-HSE-014 01 Date : 17/09/2013 Emergency Preparedness & Response Procedure Bakrie Construction PTY AND ENVIRONMENTAL MANAGEMENT SYSTEM 1.0 PURPOSE This procedure describes the planned response and preparedness to manage emergency situation caused by accident happen inside Yard or outside activities under PT. Bakrie Construction, 2.0 SCOPE This procedure applies to PT. Bakrie Construction yard and job sites. It comprises the identification of potential accident and emergency situations, definition of roles and responsibility, planned response, provision of resources and reporting of accident and emergencies. 3.0 DEFINITIONS Emergency Any abnormal and dangerous and/or environmentally threatening situation that requires prompts aetion to control, correct and return to a safe condition. Types of Emergencies include ° Fire © Explosion © Oilspills © Gas leak (Toxic or flammable) © Force major © Civil disturbances © External events (Loss of services, emergency in neighboring facility) Warden ‘A person who is responsible to co-operate with the Emergency Coordinator through assisting in the management of an emergency situation, until the situation is under control Coordinator ‘A person who is responsible for the organization of all elements to bring an emergency situation under control. 4.0 RESPONSIBILITY 4.1 CEO, CFTO and General Manager Operations 4.1.1 Provide resources and facilities needed to manage emergency situation inside Yard. HSE Manager 4.2.1 Provide procedure and guidelines related with the preparation and response in case of any emergency, includes identified of potential risk. 4.2.2 Advise Management (CEO, CFTO, GM Operations’) related with improvement related with the procedure, ERT team, and facilities needed to the subjects. 4.2.3. Provide immediate report to Management in case of any emergency accident happen, All Head Department 4.3.1 Active to support in his sub-ordinate to be dedicated as member of ERT. 4.3.2. Support ERT team to handle emergency case in the Yard. HSE Coordinator / Officer 441 442 443 Ensure the location happen incident be barricade for access. The EAP shall be reference in the Workplace, Yard or Jobsite HSE Plan. Where a potential emergency is identifying, an Emergency Action Plan shall be developing. The EAP shall include all relevant information regarding the danger to adjacent properties. 5.2.3 The action plan will described below items : Emergency Response Team and Member. Communication system Emergency Services Muster Point Mapping Site layout Diagram Exit Routes MSDS, Employee Training ‘© Emergency Resources © Emergency Procedures Post Emergency Procedures Emergency Plan Testing and Evaluation 5.3. Emergency Response Team & Member 5.3.1 ERT team will have structure organization includes Adviser, leader, Coordinator and member. See attached ERT Organization chart at appendix ~ 1. 5.3.2. The main responsibility of ERT team as follows : Response to the emergency contact & direct go to the location. Provide emergency assistance to the injurer ( victim ) Evacuate personal from incident location. Bach Department Head should ensure that nominated Wardens at department have performed their role, Communication System An effective alarm and communication system shall be installed at each workplace to allow Wardens to communicate with the Coordinator and site personnel to ensure an orderly and well executed emergency response. See at appendix to see the warden and contact number. Appendix - 1 Dedicated Warden at all department 5.5 Emergency Contact Number Emergency contact number for all BCON can be see at appendix — 2. The important number in case of emergency as follows : - Bytelp ext 132 or 141. - By radio at channel 8. 5.6 Muster Points Muster point have been designated at 3 location as follows : - Must point 1 Location in front of Clinic or Man Office. So in case Mustering all peoples at around Main Office I & Il, HSE & QA Office, Procurement Officer & warehouse, Clinic, and also portal camp office for PMT & Container must go to Muster Point 1 ~ Muster point 2. Location in front of Locker Room, So in case Mustering all peoples inside Yard Operation and Fabrication Shop, QC Office, and Client Office must go to Muster Point 2. ~ Muster point 3. In front of Mosque. So in case Mustering all peoples around camp, post security and access route must go to Muster Point 3 5.7. Mapping A location map that shows the site in relation to the local public roads including the street name and number and map reference shall form part of the Emergency Action Plan may show: © Muster area Restricted area © Work area © Medical clinic area PT. Site Layout Diagram Aste layout diagram shall be included in the EAP to show all locations of emergency resource equipment and facilities, boundaries, roadways, buildings, normal entrances and exits, alternative entrances and exits, emergency access points and any other useful information. ‘The layout diagram should include grid references. 5.9 Exit Routes Points from which the site can be exited need to be clearly marked in the EAP and kept free from obstruction and in a maintained condition. 5.10 MSDS Material Safety Data Sheets (MSDS) containing information on the reactivity of materials on site should keep in a safe, accessible location for all shifts. MSDS will be display at around hazardous chemical area. 5.11 Employee Training and Periodic Drill 5.11.1 The EAP shall be explained to all project personnel at the induction training and displayed on notice boards, canteen, meeting room, etc. 5.11.2 Training will also be provided to the nominated Warden and Coordinator in the implementation of the emergency procedures. 5.11.3 Emergency drill will be performed periodically and documented the report. Any items to be improved during the drill shall be follow up. 5.12 Emergency Resources. Emergency Resource needed to handle emergency case has to meet with potential emergency case at Yard Operation. Current available resource includes below items to anticipate : = Medical Case includes Clinic complete with 4 paramedic, doctor, first aid, ambulance, First Aid Kid, stretcher, defibrillator, personal rescue from height - Fire case includes Portable fire extinguisher with type multipurpose (ABC) = Oil spill kid - Bte PF. It should cover clean up and closeout responsibilities, statutory investigations and notification, insurance notification, post emergency report as well as the review and revision of the EAP.
