Introduction
1.1 General

The Government of India has entrusted to the National Highways Authority of India (NHAI),
under Ministry of Shipping, Road Transport and Highways (MORTH) for the development of
highways under National Highways Development Program (NHDP). The projects contemplate
enhancing the traffic capacity and safety on the selected heavily trafficked high density stretches
corridors of National highways for efficient transportation of public and goods. It is stipulated in
the Concession Agreements (CA) of the Public Private partnership (PPP) Projects that the
Concessionaire shall develop, implement an administer a surveillance and safety program for
providing a safe environment on or about the Project Highways and or shall comply with the
safety requirements set forth in Schedule ‘L’ of the CA in Development, Construction and
Operation & Maintenance Phase of the Project. It is also stipulated in the CA that NHAI shall
appoint an experienced and qualified firm or organization as a “Safety Consultant” for carrying
out safety audit of the Project Highway in accordance with the safety requirements set forth in
Schedule-L for the aforesaid Phases of Project.

1.2 PROJECT BACKGROUND

The project Corridor under consideration falls under NHDP Phase IV, a program of 4 laning to be
undertaken on Design, Built, Finance, Operate and Transfer (Toll) basis. The project corridor is
located in the state of Rajasthan and passes through Rajsamand and Udaipur districts.

The Development of Project consists of Four Laning of Gomati Chauraha - Udaipur Section of
NH-8 from Km.177.000 to Km.260.100 including service road on both sides of Main
Carriageway as indicated in CA, Schedule - B. The estimated cost of the Project is Rs. 914.5
Crore. The Project was awarded to M/s Shreenathji - Udaipur Tollway Private Limited
(Concessionaire) and the LOA was issued on 12th March 2012 and Agreement signed on 18th
April 2012. The Project construction period is 30 months and the concession period is 27 years.

M/s. Aarvee Associates Architects Engineers & Consultants Pvt. Ltd. has been appointed as
Independent Engineer (IE) of the project. Independent Engineer is mobilized on December
2012. IE has established its office (TL's Office) at H.No. 52, Jain Colony, New Bhopalpura, Behind
Mataji Mandir, Udaipur for providing effective services for project work in between Km. 177.000
to Km. 260.100.

2.0 Road Safety Audit

Road Safety Inspection was conducted from 22 May 2014 to 26 May 2014 by M/s Aarvee
Associates represented by the following professionals:

Mr. Rajan Kumar, Resident Engineer

A meeting was conducted with concessionaire at their camp office on 26 May 2014. Mr A.N
Batavi (CPM) and other senior professionals had attended the meeting. The agenda of the
meeting is to discuss the safety measures to be taken during the construction stages.

Observations

During the site visit it was informed by concessionaire officials that about 90% of the detailed
design drawings were finalized. The inspection team visited the works zone include
construction of Highway works (C&G, Earthworks in Embankment and sub grade, GSB, WMM,
DBM), Structure works and Tunneling. It was observed that about 30% of the physical work is
completed in overall and the construction progress is good as on date. The following areas of
construction were reviewed for safety and observations were recorded below:

1. Road Construction works


• DBM- About 37 km completed
• Earthworks, GSB, WMM in progress
2. Construction of Tunnel
• Underground excavation is in progress
• Open cut in progress
3. Elevated Flyovers at Nathdwara & Rajsamudh
• Girder casting and erection in progress
4. Other Structural works including Major and Minor Bridges
• Work in progress at 17 minor bridges and 2 major bridges
5. Proposed Median Openings
6. Site Camp and Plant locations

2.1 Highway Construction works

Observations

• Heavy machinery working nearer to highway


• Deep excavated areas not protected
• Lack of warning signs
• Improper barricading
• Improper access control to work zones
• Observed few Redundant signs

• Traffic riding on WMM surface


• Diversions areas are improper and badly maintained
• Non uniformity in the methods of traffic control and placement of signs
• Traffic operation and provisions found be unsafe and inefficient
• Road poorly maintained at some project stretches

Recommendations

• The Concessionaire shall refer to the requirements specified in Section 100 of


MORTH specifications- particularly Clause 101, 105 and 112 and IRC: SP:55
(Guidelines on safety in Road Construction), IRC:67 and IRC 35, Road Traffic and
Work Zone Safety Manual, Good Industry Practice and provisions of Building and
Other Construction Workers Acts/ Rules
• Adequate protection to the live traffic to be ensured while operating heavy
machinery nearer to the traffic to prevent run over back over
• Provide traffic control devices – the signs, cones, barriers, flashing lamps placed
adjacent to roads to regulate, warn and guide road users
• Work Zone safety – Provide adequate measures that control driver behavior
through construction zones
• Proper illumination for visibility at night
• At the Intersection, where the diversion starts and the road is fully closed, the use
of permanent type barricade (Type III) is recommended
• Excavation deeper than 1.2m, Type IV barricading shall be provided
• All signages must comply with the specifications in current guidelines and IRC:
67-2012. Provide sufficient signs at regular intervals to warn drivers of
construction zone, speed limit and two-way traffic. Few redundant signs observed
at site shall be removed
• Conduct Road maintenance works at stretches ( especially at diversions) of poor
riding quality. No traffic shall be allowed to ply on WMM surface.

2.2 Tunneling works

Observations

• No access control to site


• No Provision for fire extinguisher
• Excessive dust emission
• Improper Personal Protective Equipments
• No information and Alarm Equipment
• No Tunnel Ventilation

Recommendations

• The Concessionaire shall provide adequate Personal preventive equipments which

are mandatory for tunnel works. This must be provided to all workers,
supervisors, Engineers and visitors
- Hard Hat or Helmets
- Goggles for eye protection
- Respiratory protection like nose masks
- Ear protection equipments
- High visibility fluorescent colour safety vests
• Access Control to site
- Controlled by entry/ exit system
- Recording the personal/ vehicles entry with a register book
• Replace “Helmet Zone” warning board to “PPE Zone” board
• Provide water sprinkler system
• Install fire extinguisher conform to IS 2190:1992,. All workers, and supervisors
shall be trained on operating the fire extinguisher
• Electrical tools to be double insulated
• Install fire detector and emergency alarm equipment
• Provide lighting equipment for power failure
• Escape and Guidance facilities; Guide board – direct road users the evacuation
route
• Provision for Tunnel Ventilation – To provide fresh air so as to reduce the effects of
contaminants so that they are within the permissible limit both during
construction and operation stage.
• Adopt water sprinkler system in case of excessive dust emission

2.3 Elevated Flyovers at Nathdwara and Rajsamand

• No pedestrian path and crossing


• No Speed reducing measures
• Unsatisfactory barrier between work zone and traffic
• Workers working at height unsafely
• Improper PPE's
• Insufficient warning and alert signs
• Poor House keeping

Recommendations

• The Concessionaire shall provide adequate Personal preventive equipments for


workers, supervisors and drivers which are visible during night hours.
• Adopt proper precaution for workers working at height. Provision shall be made
by fencing, by safety belts or otherwise to ensure the safety of person
• Provide barricades which will not allow public to enter the work site. Work site
entrance will be controlled by entry/ exit gate
• Channelize pedestrian movements at heavily crowded pedestrian traffic especially
at Nathdwara
• Heavy Vehicle movements within the site entry and highway to avoid accidents

• Electrical cables at site are properly insulated and adopt proper housekeeping
measures

2.4 Other Major & Minor Structures

• No PPE's
• Children observed on site
• Working at height without precaution

Recommendations

• The Concessionaire shall provide adequate Personal preventive equipments for


workers and supervisors.
• No children below 14 years of age are allowed on site
• Ensure working at height with proper safety measures

2.5 Site camps

• First aid boxes missing


• Inadequate safety slogans
• Insufficient Fire safety measures

Recommendations

• Provide first aid boxes with first aid medicines


• Provide safety posters at all site establishments
• Ensure adequate fire safety measures

2.6 Proposed Median openings

About 44 proposed median opening locations which were reviewed for safety. The median
opening at the following chainages to be considered under strict safety measures

1. At Chainage 181.850 – Median opening at the gradient


2. At Chainage 199.950 – Median opening in sharp curve
3. At Chainage 216.00 – Median opening at straight road, which may results in back to back
collision of vehicles
4. At chainages 191.250, 216.300, 223.390, 230.720, 243.300, 248.017, 251.310 – Distance
between two opening is less than 2 km

3.0 Summary of findings and Recommendations

Safety measures provided by the concessionaire at the working zones are not comply with IRC
Standards. The Concessionaire shall take necessary steps to improve the safety measures by
providing signs, delineators, blinkers, barricades etc as per the IRC 55 7 IRC 67:2012 standards.
Following are the recommendations made:

1. Authority shall appoint a Safety consultant as specified as per clause 4 of Schedule L of


the concession agreement within 90 (Ninety) days from the date of concession
agreement.

2. The concessionaire shall take proper and vigilant measures of traffic and safety measures
at heavy urban settlement areas at the Flyovers locations viz Nathwara & Rajsamudh

3. The Concessionaire shall follow safety measures mentioned in the methodology for
tunneling operations. Special emphasis shall be given to tunnel ventilation,
communication system during emergencies and fire protection system

4. The concessionaire shall ensure regularity in placement of signs and sizes to follow as
per IRC standards

5. Barricading of the site areas shall be as requirements and functionality – It must


separates work zone and traffic flow

6. Prohibition of children below 14 years of age shall be adopted strictly

7. The Concessionaire shall adopt the following Safety awareness and precautions
- Installation of Safety posters at base camps and offices
- Tool Box meeting and safety drills
- Risk assessment and precautionary measures
- Fire Protection measures
-Compulsory disclosure of information regarding danger, detailed health and safety policy to
workers especially working at confined areas
- Enforce proper PPE standards at site
- Ensure electrical and mechanical safety and checking of required licenses
- Recording of accidents and near misses to NHAI/ IE on Monthly Basis
-Deployment of required safety personnel, Inspection at regular basis and ensure proper
maintenance of safety measures at site.
