AFFIDAVIT

I, MA. ANNE GAIL T. MANIGSACA, of legal age, married,

Filipino, and a resident of Purok 22, Poblacion, Manticao, Misamis
Oriental, after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That I am the registered owner of JM GYM with business

address at Purok 22, Poblacion, Misamis Oriental, per
Business Permit No. 2018-06-289 issued by the local
government unit Manticao, Misamis Oriental;
2. That the above-mentioned fitness gym does not hire
employees as this business establishment is situated at our
family home;
3. That my husband, Jason S. Manigsaca and I manage the
daily operations of this business;
4. That I execute this affidavit as to the truth of the foregoing
facts and for whatever legal purpose/s this may serve;
