I, RAFAEL PEDRO CAMINS BACULIO, of legal age, married,

Filipino, and a resident of Barangay Pedro Sa Baculio, El Salvador
City, Misamis Oriental, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am the sole proprietor of RAMIGA TRAVEL AND

TOURS with business address at Barangay 9, Cagayan de
Oro City;
2. That the above-mentioned business has been registered with
the Department of Trade and Industry, per Certificate No.
01686275 that is valid from 14 March 2012 to 14 March 2017;
3. That the local government of Cagayan de Oro City has
issued a Certification indicating the Business Registration
Plate Number and that the same has been granted renewal;
4. That the travel agency has been in existence since year 2012
under my name;
5. That the agency has been conducting its services in the
aforesaid business address from the start of its operations;
6. That the agency has only one existing business address and
that it has no other branches in another barangay, city,
municipality, or province actually operating;
7. That I execute this affidavit as to the truth of the foregoing
facts and for whatever legal purpose/s this may serve;

of the said business for seven (7) years and three (3) months,
more or less;
5. That as the General Manager, I have been overseeing all
administrative functions in the business, including but not
limited to the following, leading and directing employees,
managing both the revenue and cost elements, and delegating
administrative tasks, such as accounting, paperwork and payroll;
6. That the agency has been conducting its services in the aforesaid
business address from the start of its operations;
7. That I execute this affidavit as to the truth of the foregoing facts
and for whatever legal purpose/s this may serve;
