Individual Application

1. Photocopy of Customer’s Identity Card (IC)

2. Proof of Premises Ownership (either one):
a. Land Title OR Sales & Purchase Agreement
b. Tenancy Agreement OR Memorandum of Sub Leased (for Tenant)
3. Joint Ownership of Premises:
a. Letter of Declaration OR Letter of Consent OR Power of Attorney (either one)
b. Photocopy of Identity Card (IC) of the Joint-Owners

Company Application

1. Photocopy of Company’s Representative Identity Card (IC)

2. Letter of Appointment as Authorized Signatory on behalf of Company (with Company’s letterhead
and signed by one of the Director/Partner)
3. Proof of Premises Ownership (either one):
a. Land Title OR Sales & Purchase Agreement
b. Tenancy Agreement OR Memorandum of Sub Leased (for Tenant)
4. Photocopy of Trading License (whichever applicable):
(for validation of company registered under Companies Commission of Malaysia)
a. Borang I – for Sole Proprietorship/Partnership Company
b. Borang 9 (Certificate of Incorporation) – for Private/Public Limited Company

Other Documents (To Be Provided by Electrical Consultant/Contractor)

1. Schematic Diagram (stamped and signed by Electrical Consultant or Contractor)

2. SESCO’s Approval Letter for Electrical Drawing AND/OR Centralized Meter Cabinet Drawing
AND/OR MSB Drawing (whichever applicable)
3. Site/Locality Plan of the Premises, Meter Position, MSB and Substation (whichever applicable)
4. Paid Connection Charges (CC) Bill (for Capital Jobs)
5. Supervision and Completion/Test Certificate (Form EIU/ST/1.0)

1. Manufacturer’s MSB Drawing and Factory Test Result
