AFFIDAVIT/AUTHORIZATION

I, ELNORA BANTILING, of legal age,married to Benjie Bantiling, Filipino and a

resident of Brgy, Damoong, Janiuay , Iloilo, Philippines, after having sworn to in accordance
with law, do hereby depose and say:

1. That I am the applicant for electric connection/membership at ILECO II, Pototan, Iloilo
for the purpose of having electricity to my residential house standing/constructed on the
undivided portion of a parcel of land (Lot No. ________ – ____),owned by the Locsin
family, situated at Brgy, Damoong, Janiuay , Iloilo, per Affidavit of Authorization
executed by _________________________ in my favor , under Doc. No.____ ;Page No.
_____;Book No._____; Series of 2020 of the Notary Public Atty. Mayen Mar E.
Gulmatico.

2. That as the applicant thereof, I, do hereby appoint, designate, constitute, and

authorized HAYDEE NILLOS, single, of legal age, Filipino and a resident of Aquino
Nobleza Street, Janiuay, Iloilo, Philippines to represent me, , file, submit, process, pay
electric meter and other necessary charges, and follow up the papers for the installation
of the electric lines for electric consumption/electricity of residential house,
standing/constructed on the undivided portion of the above – mention lot at the
following: Janiuay Municipal Assessor’s Office, Municipal Engineer’s Office, Bureau of
Fire Station, Office of the ILECO II, Pototan, Iloilo and/or any
office/agency/private/government concerned;

3. That furthermore, she is authorized to sign and execute any documents and papers
relating thereto;

4. That I execute this Affidavit/Authorization to attest to the truth of the foregoing

statements.
