Formative Evaluation Plan

For my formative evaluation, I will interview a seventh grade math student at a similar level as
my eighth grade students, a 5th grade math and science teacher, and a 7th and 8th grade
science teacher. Both instructors were part of developing the problem solving framework in
our Math/Science PLC. I would like to share this lesson and the resources with my PLC for the
upcoming school year, so I thought it would be beneficial to get the two instructors'
perspectives. Below is a table of questions I will ask the student and instructors in the
one-to-one evaluations.

For the small group evaluation, I will ask for verbal feedback at the end of the lesson. I will also
take notes of how students are using the foldable throughout the lesson. This lesson will be
taught in my lowest on-level eighth grade math class. There are 21 students in the class. The
lesson will be taught over the course of two days.
