RNSG 1424 Acid-Base Interpretation

Please provide your interpretation of the following acid-base values and determine compensation.

1. pH 7.25 ↓ Co2 40 ↔ HCO3=16 ↓ Metabolic Acidosis

2. pH 7.40 ↔ Co2 37 ↔ HCO3=26↔ Normal ABG

3. pH 7.55 ↑ Co2 28 ↓ HCO3=24↔ Respiratory Alkalosis

4. pH 7.45 ↔ Co2 47 ↑ HCO3=30↑ Resp. Alkalosis w/ Meta. Comp

5. pH 7.60 ↑ Co2 44 ↔ HCO3=38↑ Metabolic Alkalosis

Please provide your interpretation with determination of compensation and interventions based on the results.

6. pH 7.31 ↓ CO2 50 mm Hg ↑ HCO3- 22 mEq/L↔

Interpretation: Respiratory Acidosis
Intervention based on result: Improve ventilation by administration of oxygen or bronchodilators

7. PH 7.31↓ CO2 44 mmHg ↔ HCO3 20 mEq/L ↓

Interpretation: Metabolic Acidosis
Intervention based on result: Administration of bicarbonate I.V. as ordered

8. PH 7.46 ↑ CO2 32mmHg ↓ HCO3- 23mEq/L↔

Interpretation: Respiratory Alkalosis
Intervention based on result: Slow the breathing rate and/or breathe into a paper bag to rebreathe CO2

Based on the scenario, provide your interpretation with determination of compensation and interventions based on
the results.

9. A 23 year old man was found to be apneic and unresponsive in the post anesthesia care unit (PACU)
following reconstructive knee surgery. About 30 minutes earlier he received 25 mg of IV morphine for pain
relief. While he is being assessed and resuscitated, an arterial blood gas (ABG) was taken:

pH    7.08↓ CO2  80 mmHg ↑ HCO3     23 mEq/L↔

Interpretation: Respiratory Acidosis

Intervention based on result: Goal is to improve breathing. If severely apneic, mechanical ventilation is
needed.

10. A 26 – year old woman is undergoing treatment for frequent panic attacks. The attacks are accompanied by
hyperventilation, a racing heartbeat, dizziness, feelings of “unreality” and tingling in the hands. In one
particularly severe attack, when taken to the emergency department, an arterial blood gas (ABG) was taken:

pH   7.52↑ CO2      26 mmHg↓ HCO3     22 mEq/L ↔

Interpretation: Respiratory Alkalosis

Intervention based on result: Rebreathing in a paper bag helps retain CO2. Encourage the patient to slow
the breathing. Patient may also be ordered an anti-anxiety drug to calm and normalize the breathing
