    PROMISSORY LETTER

    Dear Director General:

    This is in connection with our online application for License To Operate (LTO).

    We would like to inform your good office that we cannot submit the Licensing Certificate or
    the slip issued by the FDA Academy on the ground that we have not yet attended the said
    seminar because of the enhanced quarantine being implemented at the moment.

    Please accept this letter as our commitment to attend the Licensing Seminar once the situation
    gets back to normal and the government lifted the quarantine period.
