KNOW ALL MEN BY THESE PRESENTS:

I, JOANNA PRADO-FORTIN, of legal age, Married, and with residence and postal
address at Barangay Poblacion Urbiztondo, Pangasinan, is the MORTGAGEE in that
Real Estate Mortgage thru DIONISIO DE VERA executed by AMADO B. SARMIENTO
on 22nd of February 2018 and noted at the REAL STATEMENT MORTGAGE witnessed
by the Punong Barangay of Barangay Malibong Urbiztondo, Pangasinan;

That having received the full amount of ONE HUNDRED TWENTY THOUSAND
PESOS (P120,000.00), Philippine Currency together with accrued interest from
KATHLYN SARMIENTO-DE VERA the niece of the said Mortgagor AMADO B.
SARMIENTO, I do hereby forever RELEASE, DISCHARGE, and CANCEL the
Mortgage mentioned above.
