THIS AGREEMENT FOR LINE UP / SELECTION PURPOSES is entered by and between

between:

PHIL ASSIST LIFE MANPOWER CORPORATION (PAMC) a corporation organized and

existing under and by virtue of the laws of the Republic of the Philippines with its place of
business address at Room 201, 202 & 203 CYA Corporate Center No. 282 EDSA Extension
Pasay City hereinafter referred as the SENDING ORGANIZATION .

AND

Mr/Ms. ____________________________________________________ Filipino citizen of

legal age, with postal address at_______________________________________________
_________________________________________________________________________
________, hereinafter referred as the CONDITIONAL SELECTED TECHNICAL INTERN
TRAINEE (CARE WORKER CATEGORY).

WITNESSETH THAT:

1. That I am one of the conditional selected Technical intern trainee with the Sending
Organization’s Tie-up Accepting Organization in Japan: Asia Exchange Association
for Industrial Technology;
2. That as one of the Conditional Selected Technical Intern Trainee (Care Worker
Category) I fully understand and accepted that I must passed the N4 Level of the
Japanese Language Proficiency Test or its practical test equivalence in accordance
to the Department Order N0.188-B Series of 2018, Article II, No. 5;
3. That my status as Conditional Selected Technical Intern Trainee will be automatically
cancelled if I failed to pass the N4 Level of the Japanese Language Proficiency Test
or its practical test equivalence;
4. That, the selection done last December 10, 2019 is not a guarantee of my training
with Asia Exchange Association for Industrial Technology (Accepting Organization in
Japan) because as stated I have to pass the N4level to gain full acceptance by said
association.
