account number(s) – for Resellers only
• VAT identification number (for EU companies)
• US Federal Tax ID number (required for US companies)
• US IRS Form W9 (required for US companies)
• DUNS number (optional)
• Existing Cisco User ID – user will need a CCO ID to
register/renew for partnership
• Company shareholder information (if not publicly
traded)
