JOB DESCRIPTION

1. Interview watchers/relatives about patient personal data upon entering Emergency

Department.
2. Vital signs are accurately monitored and recorded within the 8 hours shift (with TLC).

3. Responsible in Entering of Patient information using Hospital Operation Management

Information System (HOMIS).

4. Responsible in Entering of Patient history, vital signs and complaints, using Hospital
Operation Management Information System (HOMIS).

5. Responsible in Discharging of patient after Treatment using Hospital Operation

Management Information System (HOMIS).

6. Responsible for making Laboratory Request and Radiology Request, order by the ROD's

7. Answers Telephone calls in or out of the Hospital.

8. Responsible in Requesting Office supplies for EMD and ED at the Supply office.

9. Retrieve Heath record at the Records section.

10. In charge in Keeping and Updating ISO Documents.

11. Responsible in Submitting DTR's and Monthly Attendance Report (MRA) at the HRM
Office.

12. Receive Interoffice Communication, and other Communication Letters concerning

Emergency Medicine Department.

13. Responsible in Maintaining the cleanliness and orderliness of EMD Office.

 14. Writes and Log all transferred patients from other Hospital and Other Health care
facilities/Private Clinics in the Referral Logbook.
15. Encoding PES and other Documents related to EMD.

16. Reports and informed Management Information System (MIS) Office about HOMIS
connection and other problem and complaints.

17. Linens are neatly folded and safely kept within 15 minutes upon receiving.

18. Encoded log sheets properly as to prescribed format before the end of the month
