7. Personal Particulars form- attach copies of NHIF and NSSF Cards
8. G.P. 7 (Indicate Bank and Branch Clearly attach copy of banker’s card
9. Next of Kin form (G.P 25)
10. Declaration Form (Secrets Act)
11. Medical Certificate (G.P. 69) signed by a qualified Government Medical Officer and
stamped – Completed in Duplicate
12. Registration Certificate from the Boards/Councils
13. Valid Practicing License
14. PSC2 Form – revised 2016 dully filled and signed – to be downloaded from PSC
website
