DOCUMENTS TO SUPPORT

AND TO BOLSTER MALASAKIT’S PRESENT PETITION FOR


REGISTRATION.1

The following documents are imperative requirement to support the


petition for registration:

a) CONSTITUTION and BY-LAWS of MALASAKIT (ANNEX A) as a sectoral


organization seeking registration under the party-list system of representation.
However, ARTICLES OF INCORPORATION , by-laws and Certificate of Registration
issued by the Securities and Exchange Commission (SEC) are not submitted with
the instant Petition since MALASAKIT did not undergo registration with the
Security and Exchange Commission

b) MALASAKIT’S PARTY PLATFORMS and PROGRAMS OF GOVERNMENT


(ANNEX B);

c) OFFICIAL LIST OF MALASAKIT’S OFFICERS (ANNEX C) As duly certified by the


Party President, the List of the respective Regional, Provincial, City and Municipal
Chapters as well as the OFFICIAL LIST OF MEMBERS (ANNEX D) to prove that the
majority of the members and officers of MALASAKIT truly all belong to the sector
that MALASAKIT seeks to represent or, at the very least a majority of the
members of sectoral parties or organizations that lack "well-defined political
constituencies" must belong to the sector they represent. A majority of the
members of MALASAKIT (as a sectoral organization that lack "well-defined
political constituencies") must belong to the sector they represent. The nominees
of sectoral organizations that represent the "marginalized and

immediately succeeding national and local elections,

e) OFFICIAL LIST of MALASAKIT’S FIVE NOMINEES AND LIST OF SUBSTITUTE


NOMINEES2 (Respectively marked as ANNEXES F and G).

f) MALASAKIT’S PUBLIC SERVICE ACTIVITIES and TRACK RECORD SUMMARY


(hereafter, ANNEX H) to show that MALASAKIT represents the interest and seeks
to uplift the welfare of the Novo Ecijanos, the sector which it primarily wishes
and seeks to represent;

g) COALITION AGREEMENT (ANNEX I), if any, and the detailed list of affiliates
comprising the coalition, including the signed coalition agreement;

h) AFFIDAVITS/SWORN STATEMENTS (ANNEX J) of officers and members of the


respective chapters (Provincial, City and Municipal Chapters) to substantiate and
submit as proof/s of existence of MALASAKIT in the areas (Nueva Ecija and Region
III) where MALASAKIT AS A SECTORAL ORGANIZATION is claiming representation;
and
