I, PATRICK NORMAN C. DEMA-ALA, of legal age, Filipino

and a resident of 486 Quezon Blvd., Panabo City, Philippines, after
having been duly sworn to in accordance with law, hereby depose
and say:

1. That I am presently employed as driver I of the Office of

Philippine Councilor’s League, Sangguniang Panlalawigan, Provincial
Capitol, Tagum City under Board Member Allan R. Dujali;

2. That as driver I was entrusted one (l) unit motor vehicle

registered in the name of the Province of Davao del Norte,
particularly described, as follows, to wit:
