I, the undersigned name is sending you this letter as notification that I am allowing /authorizing
the bearer of this letter, MOHAMMAD JAMIL M. AMINO my co-teacher to transact and
claim for the authentication of my PRC ID in your office.

I won’t be able to transact and claim it personally because of an important reason that is why I
have asked him to bring this letter together with my identification card as valid proof. I am also
giving him the right to sign any papers that has connection with transacting and claiming of the
authentication of my PRC ID.
