AFFIDAVIT

having been duly sworn to in accordance with law, hereby depose and state, that:

1. I am a teacher and the manager of XXX Canteen located in Barangay XXXX.

2. I registered the above-mentioned canteen with the Business Permit Licensing

Office of XXXXX sometime in 2018. However, we did not operate or open said
canteen the whole year of 2019 due to the fact that I was assigned by are then
newly appointed principal to handle the Grade II section.

3. I am executing this Affidavit to attest to the truth of all the foregoing statements
and for all legal intents and purposes this may serve.
