‘’Smartphone as a Learning Tool: Its Effect on the Academic Performance of the Senior
High School Students’’. This study was explained to me by _____________________, a
researcher from STEM of Philippine Advent College Academy.
I understand that I will be a respondent in this research study that will take me about an
hour. I will face no risk by taking part in the study. My participation is voluntary, and I can
change my mind at any time. About 50 students will take part in the study.
The researchers conducting the study will not reveal my name to anyone, and my name
will not appear in any reports on the study. I was informed that if I have questions, I can call
