Good day.

This is in connection with the _____________________________________. We are

formally writing to your good office to ask for a reconsideration regarding the abovementioned
resolution. The said resolution provides among others that “ xxx, the deceased member should
receive at least twenty-five (25%) of the insurance claim and only 75% will be paid to the
delinquent loan where he acted as co-maker; further, that the said policy will be incorporated in
the new Promissory Note. xxx.”

we are asking you favorable consideration that we will be able to receive at least fifty
percent (50%) of her insurance claim and the other fifty (50%) will be paid to the delinquent loan
where she acted as co-maker, since we need the insurance claim to pay for her hospital
expenses before her death and other funeral expenses.
