Good Day!

As the formation year is about to end, it has been the duty of our Seminary priest-formators to evaluate
your son-seminarians. They make sure that before they evaluate, all seminarians must first make their
own evaluation of themselves basing on the four aspects of formation and furnish a copy to be given to
the formators. Due to this pandemic, we are challenged of properly communicating to all the
seminarians that is why we are asking your cooperation to please answer this survey faithfully and
honestly:

1. Do you have a Wi-Fi connection at home?

2. Are you able to establish proper signal connection through mobile data?

3. Do you have laptop/PC/mobile application at home which can be able to make a Word
Document?

4. Do you have other suggestions? (please specify)

This is relative to seminarian’s readiness to digital learning. Results of the survey will be the Seminary’s
Administration for the final decision about this matter
