TRANSFER OF PENSION

PERSON ELIGIBLE:

Surviving beneficiary of the deceased retired AFP Pensioner. Surviving beneficiary are
classified as surviving spouses, parents of unmarried pensioner and children below 21
years old of the deceased AFP pensioner.

REQUIREMENTS:

For personally appeared pensioners:

1. Accomplished Pensioner Update Form (PUF) Original Form


2. OTAG Retirement Order of the Principal Pensioner (retiree) Original Form
3. PSA (formerly NSO) Death Certificate. Original Form
4. Certificate of Beneficiaries from NRD, OTAG. Original Form
5. PSA Marriage Contract with OR, if the deceased retiree is married
 PSA CRS Nr. 5 (Advisory on Marriages – Husband & Wife)
6. PSA CRS Nr. 4 (CENOMAR) with OR, if the deceased retiree is single
7. AFP Beneficiary ID. Photocopy
8. Pension Account opened with any Government Servicing Banks Photocopy
9. PSA Birth Certificate of all children
10. Accomplished Letter of Introduction duly signed by the Bank Manager and C,
DRB AFPFC. Original Form
11. Retiree’s ID issued by OTAG Photocopy

Additional requirements for non-appearance:

12. Duly accomplished Proof of Life Form (Post card-sized whole body picture
holding current broad newspaper) Original Form

Headquarters, AFPPGMC, Camp General Emilio Aguinaldo, Quezon City,
1110. You can also visit AFPPGMC’s Satellite Offices located nationwide.

Reminders:

Submission of requirements can be made immediately after the death of the


Principal Pensioner (Retiree) to be accepted and scheduled for processing in
the Pension Gains Payroll report of changes.
