7.       Oath of Service (attach 1 original copy)
8.       BIR Form 1902/2305 (attach 1 original copy)
9.       DBP ATM Account Number
10.   Pag-ibig Number
11.   Philhealth Member Data Record (MDR)
12.   GSIS Member’s Information Sheet (attach 1 original copy) / BP No. for transferees
13. Certificate of Last Payment (for transferees)
