General Examination:

The patient was lying at 45 degrees, comfortable with a canula on her left hand and name tag
on her right. She was alert to time, place and person.
On Scalp there was no evidence of Drandruff/Psoarsis
. The scar was measured to be 7 cm long. There was no suprapubic pain felt. Fundal height
was 39 cm at 39 weeks.
Using leopolds grip: On fundal grip the buttocks were felt as soft & irregular.
Lateral grip: Back is felt on the left as a smooth curve & the limbs felt on the right as
irregular knobs. Pelvic Grip: The head is felt and its not engaged.
A single term fetus in an oblique lie in cephalic presentation, back on the left side and limbs
on the right, head isn’t engaged, adequate liqor volume, estimated baby weight is normal.
On ausculatation the fetal heart sound was heard at the left iliac fossa.

days of period. She has no blood clots clots. There is no history of dysmenorrhea, post-coital
bleeding and intermenstrual bleeding.

The first day of last normal menstrual period is 25th October 2019
EDD is 2nd august 2020.

HOPI
It is an unplanned pregnancy. A UPT was done at home a month after missing her period
and it was inconclusive. Pregnancy was confirmed 5 months later, when she felt fetal
movement, she went to private clinic/GP and ultrasound was done confirming it is a
singleton and intrauterine pregnancy.
She had no symptoms indicating she was pregnant in both her first and second trimester.
No nausea or vomiting. Her booking was donethe following day at kk kampung pandan.
At booking her weight was 55kg height is 160 cm and bp is 116/76. Her blood group was B+,
unsure of her Hb level but it was within normal limits and her infective screening for was
normal.
The first movement was felt at 5 months. Mggt was also done in the booking. She claims it
was done only once. The results were normal.
Her urine investigations at booking was also normal.
Her antenatal checkup was twice a month until 7 months of pregnancy then it once every
week. She is unsure of her color coding.
At 32 weeks she was told that her child has IUGR and was planned for an elective induction
for when she is 37 weeks.
She had induction of labour today morning. She did not experience any contractions,show
and leaking. Her fetal movement is normal where she usually gets 10 movements by 9pm.
Urine investigation was done upon admission was normal.
