AFFIDAVIT

I, KAINAT LATIF D/O MUHAMMAD LATIF, Permanently resident of House No,

138, Street No. 02, Sector E, Akhter colony Karachi, do hereby declare on oath and
solemnly affirm as hereunder:-
1. That, as a Parent Guardian, I accept that if my daughter does not fulfill the
requirement of 75% attendance in each subject, as required by the college and
Board of Intermediate Educations Karachi/ University of Karachi, and does not
appear for all college Tests and Examinations. She shall forfeit her right to be
enrolled by the college in the said BOARD/UNIVERSITY EXAMINATION and
shall also forfeit the right to receive her relevant ADMIT CARD.
2. That, having clearly read all the requirements and rules for payment of college
fees by the dates required, I affirm that I shall pay the required fees on the dates
scheduled and any default in payment will give the college the absolute right to
strike my daughter's name off the college rolls and she shall cease her to be a
student of the college with immediate effect.

3. That I / am fully conversant with, and accept as binding on me, the college rule
that fees once paid are non-refundable. I also accept that I will not attempt to
bring pressure to bear or attempt any litigation in the matter of fees / or fees
reduction.
4. That in the event of injury, loss or damage sustained by my daughter during
course of studies within the college premises, I shall have no claim for damages
or compensation whatsoever against the college and also do indemnity the
college, its PRINCIPAL , FACULTY MEMBERS, OFFICERS, and STAFF
against all such CHARGES / CLAIMS.
5. That whatever I have stated above is true and correct to the best of my knowledge
or belief and nothing has been concealed therein.
