OF ONE AND THE SAME PERSON

MARIA REGINA SOCORRO MAGBANUA RAMOS, of legal age, single,

Filipino citizen, and residing at No. 546 San Isidro Sr., Hagonoy Bulacan, after
having been duly sworn to in accordance with law, do hereby depose and say
that:

She is the third child of Domingo Francisco Ramos and Dolores

Magbanua Ramos;

She was born on May 26, 1981 in Malolos Bulacan;y

In her Certificate of Live Birth she was registered as MARIA REGINA

SOCORRO MAGBANUA RAMOS;

When she started schooling she has always used the name MARIA
REGINA SOCORRO MAGBANUA RAMOS;

In her Voter’s Identification Card, Tax Identification Card and other

government issued documents, she was known as MARIA REGINA SOCORRO
MAGBANUA RAMOS;

In her Social Security System record, she was known as MARIA REGINA
SOCORRO MAGBANUA RAMOS, however, in its duly issued SSS Identification
Card her name was registered as MARIA REGINA SO MAGBANUA RAMOS
due to lack of space;

This affidavit is being executed to attest to the truth of the foregoing facts,
to establish that MARIA REGINA SOCORRO MAGBANUA RAMOS and MARIA
REGINA SO MAGBANUA RAMOS refer to one and the same person and for the
purpose of visa application.
