7/22/2020 – IHuman, Vaginal bleeding & abdominal cramping, 82%

This scenario is about 26-year-old Melissa Frank who is 34 weeks pregnant and presents to the
ED after experiencing a “fall” at home and landing on her left side. Patient had been
experiencing bleeding and abdominal cramping for 30 minutes prior to arrival. I believe that I
demonstrated my best evidence-based practice when I reviewed her chart and then spoke to her
and some information did not appear similar to the story she was telling me as well as she tested
positive for cocaine use in the best 7 days; I suspected potential domestic violence. After asking
appropriate questions, she later admitted her boyfriend was angry with her and kicked her in her
stomach prior to her falling; she did not admit to cocaine use during my assessment. My strength
in this scenario was appropriately and thoroughly going through my physical exam and
determining additional evidence of domestic violence history prior to today’s incident. My
weakness was the prioritization of when actions and treatment should be completed. The ATI
case study on polypharmacy that I did today helps me with practicing the importance of
reconciling patient records to make sure there aren’t multiple prescriptions or medications that
interact.
