This Annex should be filled out for each type of training and each level the bidder is bidding for
separately
Ex: if the bidder is submitting a proposal for a training in cooking basic and advance, then they should
submit two forms for each training (Cooking - basic and cooking - advanced)
Please fill out all the required information in the different sections
Please list all skills that the individual will learn through the training and associated number of days of training
for each skill between brackets
