LICENSURE EXAMINATION & REGISTRATION INFORMATION SYSTEM

SPLE APPLICATION
1. Before you start, you must have the following on hand:

- Your e-mail address (with password)

- Your mobile number (ex. 09271234567)

- Personal data (including date of graduation)

- ID photo in .jpg and .png format saved on your computer, phone or


tablet (You can request for an e-file if you have your ID photo taken at a
photo studio. Maximum of 2MB)

- Passport
2. At the PRC Homepage, Click the [Online Application] eService and .prc.gov.ph and point the
mouse cursor to the Online Application then select [SPLE Applicants].
3. If you are using this for the first time, click on the REGISTER tab. Read the Terms of Service
(ToS) CAREFULLY and click on the button at the end of the
ToS if you wish to continue.
3A. Fill out the Registration form. You MUST create your OWN account using your OWN email
address. You are allowed to use ONLY ONE VALID EMAIL ACCOUNT PER
USER/APPLICANT. Complete all fields where applicable then click the [REGISTER] button.
4. If you have previously registered online or finished signing up, enter your email address and
password, then click SIGN IN.
5. Create your profile. To make sure that information is accurate, you must fill out these forms
yourself. Make sure the First & Last name is exactly the same as printed on your existing
PRC ID. Where appears, search with the drop down menu. Do not type in the
information.

6. After filling out your profile. Click on the SAVE INFORMATION button..
7. Double check your information before you click on the YES button. (You will not be allowed to
change your name after clicking YES.)
8. Your application details will be displayed. You may still edit your information (except for your
name) by clicking on the EDIT button. Click the icon to upload your photo.

9. READ the photo requirements carefully. To avoid delay or disapproval, please upload good
quality ID photo. Click the [PROCEED] button.
10. Upload your picture via your computer or phone directory.
(Photo size must not be more than 300 x 300 dimension. If photo is not in accordance w/
specifications, you will be required to change the photo you attached.)
11. To continue, click SELECT TRANSACTION.

12. Select EXAMINATION.

Click Here
13. Select the Name of Examination, Examination Type, Date of Examination and Place of
Examination, then click the PROCEED button.

14. Check the Examination Details then click the PROCEED button.

15. Confirm the Examination Details then click the SUBMIT button.
16. CLICK INPUT RECEIPT DETAILS button to input Transaction Receipt number and
Transaction date.
17. AFTER PAYMENT, enter Receipt number (ePP Reference Number) and Receipt Date
(Payment date). Wait for 1-2 days after payment date before printing the Application Form.
Click the SUBMIT button.
18. Verify your status at EXISTING TRANSACTIONS Tab.
19. ONCE PAYMENT IS VERIFIED, Click PRINT DOCUMENT button.
20. Click to print the Application Form.
21. Click your name on the upper right corner of the screen and click the LOGOUT button.

REMINDER
Remember: always LOG OUT at the end of your online session especially when using public
computers (Internet and cyber cafes). This is to prevent others from accessing your account and
to protect yourself from fraud, identity theft and other malicious intent.
