Waiver

_____________________________________________________, after having sworn to in accordance with law, hereby
depose and state:

1. That I voluntarily enlisted myself to take part in the Singles’ Camp organized by the members of
Victory Christian Fellowship of Aklan, to be held on May 17 and 18, 2019;
2. That I acknowledge that this event will involve travelling by land and by water, and other activities
that may be physically taxing to some extent;
3. That I certify that I am physically fit, have sufficiently prepared for participation in this event and
have not been advised otherwise by a qualified medical person;
4. That I shall be responsible for my own actions during the whole duration of the camp, including its
travel period, and that in the absence of fault or inexcusable negligence on the part of the camp
organizers, the latter and/or Victory Christian Fellowship shall not be made liable for any
untoward incident that may happen to me during the event, or immediately prior or thereafter.
