AFFIDAVIT OF BEING SINGLE

I, CARLO PAULO AQUINO CADUT, of legal age, single, Filipino and a

resident of 73 Upper Malvar Street, Trancovile Baguio City after having
been duly sworn to in accordance with law, hereby depose and say:

That I am applying in the Military Service as trainee in

the Philippine Army;

That I was born on August 18 1991 and one of the

children of spouses Elpedio Cadut Jr and Felomina Cadut;

That I am still single, as I have never contracted any

marriage with another person up to the present;

That I am executing this affidavit to attest to the truth of

the above-statement for whatever legal purposes that this
affidavit it may serve.
