Case formulation

Diagnosis:

The client has balloon phobia which means that the client avoids Balloons and is afraid or has a
fear of balloons. The source of fear is the sound of balloons popping. She refuses to touch, feel, or go
near a balloon for fear it will pop.
Psychoeducation:
You are having balloon phobia due to which you do not go near balloons due to the fear
that it will pop and you cannot bear the sound of balloon popping. Because that sound is very
frightening for you.as you have mentioned, this issue developed when you were little and you
were celebrating a birthday party and when many balloons popped at same time, it was very
fearful for you and from that time onwards, you avoid balloons. But you should not take any
tension as we will cooperate with each other and ultimately find a solution for this problem.
Treatment plan:
Education and training in a brief relaxation technique,
‘in vivo’ exposure
development of cognitive coping self-statements.
Need 8-12 sessions
Therapy:
Cognitive behavior therapy (CBT)
Target symptoms:
Fear of balloons
Avoidance of balloons
Short term goals:
Minute tasks will be given to client like watching photos or videos of balloons, just going
near and holding balloons.
Long term goals:
Long term goal simply is to eliminate her phobia of balloons and make her comfortable
situations involving balloons.
Initial phase of treatment:
First step as a therapist will be to build rapport with the client, to make the client feel
comfortable and accepted. Client will be assured of privacy and confidentiality. I will thoroughly
investigate the problem and pay attention to each aspect.
Middle phase:
I will apply the interventions of CBT like relaxation techniques, educate regarding coping
self-statements.
Follow up:
. After completion of sessions after successfully implementing treatment plan, I will call
her for follow up after one month and then after six months.
