HPDCI COVID19 PEME Guidelines

1. HealthPlus Diagnostic Clinic Inc. (HPDCI) will prioritize healthy patients for Pre-Employment
Medical Examination (PEME). If your employees have cough, fever, colds, sore throat or difficulty
of breathing 2 weeks prior to his scheduled PEME, please advise them to visit a hospital. If your
employees do not want to visit a hospital, HPDCI will entertain telephone consultation after 3 PM.
Please instruct the patients to download viber or whatsapp and contact 09175500459 for the
consultation.

2. The Clinic will practice a NO FACEMASK/NO ENTRY policy. Upon entering the building,
patients have to register their name and cellphone number to the guard. Patients are required to
sign a health declaration that he/she is not suffering from cough, fever, colds, sore throat or
difficulty of breathing 2 weeks prior to his scheduled PEME.

3. Physical distancing is a must when the patient is in the clinic! HPDCI will not allow any
companions in the clinic. Please instruct your seafarers to bring 1 pen and 1 pencil. Sharing of pen
and pencil is not allowed. The clinic can only handle 40 patients at a time. A first come first serve
basis will be implemented. For the 41st patient onwards, the clinic will text/call the patient once
another patient leaves the clinic.

4. Please email info@healthplusdiagnostics.ph and kristofrank@healthplusdiagnostics.ph to

schedule your seafarers. Wait for confirmation from HPDCI before sending your seafarers to the
clinic.
