in Nursing at the Notre Dame of Dadiangas University in 1991.

I have served as staff nurse for two (2) years at Diagan’s Clinic and Hospital, General
Santos City; two years at the Private Duty Nurse’s Association; one (1) year at General
Santos City Health Services; two (2) years at the DepEd Salimbao, ARMM; one (1) year
at Villegas Opthalmology Clinic of St. Elizabeth Hospital, General Santos City; four (4)
years at Primary Health Care Delivery Program of Notre Dame of Dadiangas University;
and ten (10) years as Public Health Nurse at the Armed Forces Southern Region in
Khamis, KSA.

Attached herewith are my Personal Data Sheet and other pertinent documents to support
my application for your perusal.
