JUDICIAL AFFIDAVIT OF
MRS. LUISA MARIA DE JESUS

I, LUISA MARIA DE JESUS, 36 years old, married, residing at Barangay

Poblacion, Zarraga, Iloilo, after having been sworn to in accordance with the law
do hereby depose and state:

That Atty. Nestor Jeremy B. Moreno is the counsel who conducted and
supervised my examination as a witness at MORENO and JALIPA LAW OFFICE,
123 Ana Rose Building, Iznart St., Iloilo City;

That I am answering the questions herein fully conscious that I do so under

oath and that I may be criminally liable for false testimony or perjury;
That the following are the questions propounded by Atty. Nestor Jeremy B.
Moreno and my answers are in ENGLISH LANGUAGE:

Q1: Do you swear to tell the truth and nothing but the truth?

A1: Yes Sir.

Q2: Can you state your personal circumstances?

A2: Yes sir. I am Luisa Maria De Jesus, 36 years old, married, residing at
Barangay Poblacion, Zarraga, Iloilo.

Q3: What is your position in the Local Government of the Municipality of

Zarraga?

A3: I am the Collecting Officer Sir.

Q4: Why are you here?

A4: To give my testimony freely and voluntarily and the same will constitute my
direct testimony in the above mentioned case.

Q5: Do you undertake to answer the questions to be asked of you, fully conscious
that you will do so under oath, and that you may face criminal liability for false
testimony or perjury?

A5: Yes sir.

Q6: What happened last January 20, 2017?

A6: It was like my every day work Sir. I reported for work at the Municipal Hall of
Zarraga, Iloilo at 8:00 o’clock in the morning of that day. Then at about 9:00
0’clock of the same day, COA officers arrived and conducted a cash examination.

Q7: Are you aware that there will be a cash examination on that day which was
conducted by COA?

A7: No, sir. I am not aware.

Q8: Did you receive any notice or information regarding the cash examination to
be conducted by COA?

A8: No, sir. I did not receive any notice or information.

Q9: On that day, were you present when the examination was conducted?

A9: Yes sir, I was on my table working on my regular paper work.

Q10: What did you do on that day?

A10: I was working on the collections I made. I was comparing the money I
received if they correspond with the receipts issued.

Q11: What happened next?

A11: There wear two persons who entered our office. They proceeded to the office
of the Municipal treasure.
Q12: What happened next?

A12: My co-worker told me they from the Commission on Audit. They were there
to conduct cash examinations.

Q13: Did the COA Auditors questioned and examined you?

A13: No, Sir. They did not examine me. It was only Mr. Vicente Queleste that they
examined that day.

Q14. What happened next?

A14. After they were done examining Mr. Vicente Queleste they went home. That
was about 12 noon when they left our main office in Zarraga.

Q15: Do you have anything else to say?

A15: No more sir.
