JUDICIAL AFFIDAVIT OF
MRS. LUISA MARIA DE JESUS
That Atty. Nestor Jeremy B. Moreno is the counsel who conducted and
supervised my examination as a witness at MORENO and JALIPA LAW OFFICE,
123 Ana Rose Building, Iznart St., Iloilo City;
Q1: Do you swear to tell the truth and nothing but the truth?
A4: To give my testimony freely and voluntarily and the same will constitute my
direct testimony in the above mentioned case.
Q5: Do you undertake to answer the questions to be asked of you, fully conscious
that you will do so under oath, and that you may face criminal liability for false
testimony or perjury?
A6: It was like my every day work Sir. I reported for work at the Municipal Hall of
Zarraga, Iloilo at 8:00 o’clock in the morning of that day. Then at about 9:00
0’clock of the same day, COA officers arrived and conducted a cash examination.
Q7: Are you aware that there will be a cash examination on that day which was
conducted by COA?
Q8: Did you receive any notice or information regarding the cash examination to
be conducted by COA?
Q9: On that day, were you present when the examination was conducted?
A10: I was working on the collections I made. I was comparing the money I
received if they correspond with the receipts issued.
A11: There wear two persons who entered our office. They proceeded to the office
of the Municipal treasure.
Q12: What happened next?
A12: My co-worker told me they from the Commission on Audit. They were there
to conduct cash examinations.
A13: No, Sir. They did not examine me. It was only Mr. Vicente Queleste that they
examined that day.
A14. After they were done examining Mr. Vicente Queleste they went home. That
was about 12 noon when they left our main office in Zarraga.
