Metformin can address both diabetes mellitus and obesity. This is an oral anti-hyperglycemic agent used
for first-line treatment of type II diabetes mellitus that acts through expanding both hepatic and
peripheral insulin sensitivity in the liver by inhibiting basal hepatic glucose production, and by increasing
glucose uptake in skeletal muscles and adipocytes. It is useful in preventing diabetes and improving
insulin resistance in conditions such as polycystic ovary syndrome. Its use was linked with weight loss,
and is one of the best choice for overweight and obese individuals at high risk for diabetes, as well as for
hypertension, hyperlipidemia, cardiovascular disease, osteoarthritis, and several other conditions.
Patients treated with metformin experienced about a one-unit reduction in BMI at the end of treatment,
as well as the fasting insulin levels, and fasting glucose levels. A study from Diabetes Prevention Program
(DPP) showed that adults who are obese, but nondiabetic, with elevated fasting blood glucose, if
prescribed with metformin, can prevent the future development of type II diabetes mellitus.
However, there are no studies that specifically address the use of metformin for the prevention of
diabetes among adolescents.
