respectfully states:

That she is one of the trusted persons and also one of the heirs, to manage all of the properties, which
one is registered in the name of the Late _________________________, covered by Transfer Certificate of
Title No. _____________, a parcel of land, (______________________________________), situated in the
_______________________________________________________. Bounded on the __.,
_____________________________________________________________________, containing an area of
_____________________________ (_____) SQUARE METERS more or less

That it is appearing that the said parcel of land was annotated for Les Pendent. Date Inscribed:
__________________________ in the said title covering the same lot. That the said Lot is not included in the
Civil Case for Lot No. ___________, thus petitioner hereby requests to the Register of Deeds of the Province of
Negros Oriental, Dumaguete City that said les pendent be cancelled;

That having complied with all the requirements which are submitted herewith, petitioner does hereby
reiterate that his request be granted.
