Online Parchment Permission Form

1. Log into your PowerSchool

Parent Portal account

Note: Accessing Online Forms is not

available through the PowerSchool Mobile
App.
If you do not have an account, please click on the
“Create Account” tab shown above.

You will need:

• Student School ID
• Date of Birth
• Student Full Name

2. Select “Access Online Forms”

(Left Navigation Panel)
 3. Select the “Parchment Parent
Permission” tab

4. Select the “Parchment

Permission Form”

Note: Please complete all required fields and be

sure to click submit.

After submission, the completed form will appear

under your “Completed Forms” section of the
main page.
