AFFIDAVIT OF UNDERTAKING

______________________, Philippines, after having been duly sworn to in accordance
with law, hereby depose and say:

1. That I am the borrower for the loan granted by FIRST STANDARD FINANCE
CORPORATION in my favor in the amount of Php____________;
2. That the same loan shall have a term of six months. The first five months
shall be interest paying and interest paying plus lump sum on the 6 th month;
3. That I voluntarily surrender my property covered by TCT/OCT No.
___________ to FIRST STANDARD FINANCE CORPORATION for safe
keeping within the period of six months;
4. That I hereto undertake to sign the necessary loan documents and all other
documents required by FIRST STANDARD in relation to the above
undertaking;
5. That I am executing this affidavit to attest to the truth to all the above facts.
