I, WILLINGTON CARAG, of legal age, Filipino citizen, married and with

residence and postal address at Bonifacio Street, Tuguegarao City, Cagayan, after
having been duly sworn in accordance with law do hereby depose and state:

1. I am the registered owner of a motorcycle with side car, more particularly

described under LTO Certificate of Registration No. 65278607 as follows:

2. I already detached the side car of the said motorcycle for personal
reasons;

of my motorcycle and for the purpose of registration of motorcycle with
the Land Transportation Office (LTO).
