PARALLEL PATHWAY LOG BOOK GUIDE

1.1. Introduction to Trainees, Heads of Departments & Educational Supervisors

To the candidate

This log book has been developed by the Ministry of Health Malaysia as a means of helping
the candidate sitting for the MRCPsych exams and the Supervisor to review the progress of
the respective candidate. It is the responsibility of the candidate to maintain and keep this log
book updated from time to time. This log book would help in the assessment of the
candidate’s training needs and it should be made available to the respective supervisor or
head of department as necessary. Although every effort has been made to make this log book
as flexible as possible, certain sections may need modifications for a candidate’s particular
interests and requirements. Do make use of the reverse side of the printed pages whenever
necessary if there is insufficient space in certain sections.

Throughout the training, the candidate should participate actively in all relevant undertakings
of their respective training centres such as the care of inpatients and outpatients, perform on
call duties where required, take part in community psychiatry visits, perform psychotherapy
sessions, participate actively in case discussions, case conferences and other educational
activities.

To Supervisors or Heads of Departments

This document should be reviewed with the candidate at the start and towards the end of their
attachment. This would help establish important training needs and educational objectives
that the candidate requires. The Ministry of Health Malaysia recommends that the candidate
has regular and stipulated face-to-face supervision over and above the usual clinical rounds
with their respective supervisors.

1.2. Check-list for individual Training Posts

At the start of the attachment, the educational objectives stated below should be established
and agreed between the candidate and the Supervisor. These objectives are not set in stone
and could be used as a basic guidance for the candidate.

General Adult Psychiatry - some of the pertinent aspects that need particular attention are:

History taking
Eliciting psychopathology & Mental state assessment
Case presentation
Knowledge and implementation of the Mental Health Act

Approach to and experience in multi-disciplinary working

1.3. Requirement of the candidate in order to attempt parts of the MRCPsych exam

1.3.1. The mandatory requirement for entry to sit the MRCPsych Paper A is:

 Any fully registered medical practitioner (For Malaysians, Full Registration with
the Malaysian Medical Council).

1.3.2. The requirement for entry to sit the MRCPsych Paper B is:

 To have 12 months experience working in a Psychiatry Department before


attempting Paper B.
 If the candidate has worked in a Psychiatric Department for 12 months or more, the
candidate may sit for the Papers in any order.

1.3.3. The requirement for entry to sit the MRCPsych CASC (Clinical Assessment of
Skills and Competencies) exam is:

 Has at least 24 months working experience in a recognised Psychiatric Unit.


 Has passed Paper A and Paper B of the MRCPsych exam.
 Has attained Competency in Child & Adolescent Psychiatry (at least 4 months’
experience/ posting) &
 Has attained Competency in any one of the forms of Psychotherapy (CBT,
Psychodynamic psychotherapy, Group therapy etc.).

1.4. The list of authorised sponsors

A candidate shall be signed-up as having fulfilled the requirements for the CASC exam by
his/ her respective Sponsor which includes:

 Training Programme Director


 Educational Supervisor
 Head of School
 Director of Medical Education (or equivalent)
 Head of Department of Psychiatry
 Consultant responsible for employee/ trainee appraisal

For further details of Pre-Membership examination requirements, kindly refer “Eligibility


Criteria and Regulations for MRCPsych Written Papers and Clinical Assessment of Skills
and Competencies (CASC)” document dated 23rd October 2015. These are subject to
change by the Royal College of Psychiatrists, U.K. Kindly refer to the RCPsych website
for latest info.

1.5. Requirement of the candidate upon passing respective papers

1.5.1. Requirement of the candidate who has passed any of the Membership Papers

On passing the first written paper (either Paper A or Paper B, as the case may be), the
candidate is required to complete the ‘Borang Pendaftaran Pegawai Perubatan yang
sedang mengikuti Ijazah Lanjutan di Laluan Paralel’ and officially inform the Medical
Development Divisional Unit of the respective hospital using this stipulated form
(Appendix 1). This is MANDATORY. A candidate who is not yet working in a
Psychiatric department at this stage, needs to be transferred to a gazetted Psychiatric
Department for further training.

The candidate is required to complete his/ her mandatory postings in the various
disciplines of Psychiatry as stipulated above. All postings in the sub-specialised fields of
Psychiatry done prior to the CASC exam would need to be 4-month postings each if they
are to be included as completed training on the Interactive CASC application form
(available online). E.g., a candidate working in a Mental Institution with a Forensic
Psychiatrist is encouraged to train in Forensic Psychiatry. For this training to count as
additional training done by the candidate (prior to the CASC) it would require to be of 4
months’ duration.

ALL MRCPsych candidates are required to do a 4-month posting in Child & Adolescent
Psychiatry under a qualified Child & Adolescent Psychiatrist prior to attempting the
CASC exam. If a Child and Adolescent Psychiatrist is available in the same state, then the
candidate would need to be posted for training under this Psychiatrist for 4 months as
stipulated by the Royal College of Psychiatrists, U.K. This transfer is an Intra-state
transfer and hence, can be effected by the Directors of the respective hospitals, the State
Health Department and the State Psychiatrist. If there is no Child and Adolescent
Psychiatrist in the same state or if the candidate chooses to do this posting in another
state, then an Inter-state transfer would come into effect and the candidate would have to
make an application for this via his State Health Department to the Medical Development
Division of the Ministry of Health, Malaysia who will consult the National Head for
Technical Services for Psychiatry before this training/ temporary transfer can be
executed. This would apply for all other Inter-state training in subspecialties of Psychiatry
that a candidate may wish to do.

While working in the department of Psychiatry, the candidate is to take part actively in all
the teaching activities of the Psychiatry Department. Proactive and keen participation is
encouraged. Interview skills training, Case Conference presentations and attendance,
Journal Club presentations and attendance, Case Management discussions with the
supervisor and/ or Psychiatrists of the department are to be done frequently and recorded
in the MOH Parallel pathway log book. Attendance at Conferences & Update workshops
is much encouraged and should be recorded in the MOH Parallel pathway log book.

1.5.2. Requirement of the candidate who has passed the CASC Examination

A candidate who has successfully passed the MRCPsych Examination is required to


inform the Medical Development Divisional Unit of his/ her hospital and the Medical
Development Division of the MOH.

As stipulated by the Medical Development Division of the MOH, the candidate is then
required to undergo 1 year of rotational postings in the various subspecialty fields of
Psychiatry.

These postings should be of 3 months’ duration or longer in the candidate’s chosen fields
of interest. The entry for these postings would be made in the ‘Year 3’ column of the
MOH Parallel pathway log book. The training would be done under a Psychiatrist sub-
specialised in the respective field. The respective Heads of Departments and Hospital
Directors would effect the transfer of hospital for these rotational postings to be done by
the candidate.

1.5.3. Requirement of the candidate upon completion of the 1-year Post-Membership


rotational postings

As per the requirements of the Ministry of Health, Malaysia, a holder of the


MRCPsych is required to have a total of 4 years experience in Psychiatry of which at
least 1 year is after passing the MRCPsych exam before the candidate can proceed with
the minimum 6 months of pre-gazettement and thus be recognized as a Psychiatrist.

A candidate who has satisfied the criteria as stipulated above, will then proceed to
commence with gazettement that would be for a period of 6 months. This could be done
in the same hospital as the last rotational posting (as long as the Department of Psychiatry
has a Psychiatrist who is more than 2 years post-qualification) or may return to the base
hospital (from which he/ she was sponsored for the MRCPsych exam).

Upon successful completion of the gazettement under the respective Psychiatrist, the
candidate is eligible to be gazetted as a Psychiatrist and also eligible for entry onto the
National Specialist Register as a General Adult Psychiatrist.
